When to use it
- A coding agent such as Claude Code runs shell commands and edits files with broad permissions, and a few of its actions (deleting data, force pushing, publishing, touching secrets or CI) should wait for a person.
- Allow and deny lists keep missing cases, because the same command can be routine in one form and destructive in another.
When not to use it
- The rule is exact, such as never running
git push --forceor never writing outside the repo. Put it in the agent's permission rules, which are certain and free. - You want to know whether the change is correct. That is a code review, not a 10 second risk call.
- Every action already waits for a person. The gate would add cost and nothing else.

Reading the result
- Routes:
askmeans a person should confirm the call first,allowmeans the gate has no objection.allownever approves anything on its own: the agent's own permission rules still apply. - Turn the call into an ask only when
routeisaskandoverallActionisauto. Any other result adds nothing, so an unsure gate stays quiet. Low false alarms matter more than catching every case: an ask on ordinary work teaches people to click through. - Start in
shadow: the gate runs and logs what it would have asked, and every call goes ahead as it would without the hook. Move it tocontrolledafter reading a week of results; there only a high bandaskinterrupts. - One risk question applies per call:
risky_commandfor Bash,risky_file_changefor Edit and Write. The other is marked not relevant, so it never blocks or lowers the run band. Thetruebar is 0.85 on purpose; lower it only if your labels show missed risks. risk_kindnames the kind of risk for the ask message. It never gates.env_fileis a code check: writing a.envfile always routes toask, whatever the model answers..env.exampleand other templates are not matched.- The command, file path and content preview go to TypeSafe. Send only the start of new content (the schema caps it at 2000 characters), and add redact paths if your commands can carry secrets you do not want to send.
- To save cost and time, skip the call in your hook code for commands you always trust, such as
git status. - Outage rule:
onUnavailableisreview. During a System One outage the gate adds nothing and the agent's own permission rules decide.
